Soleo Health is seeking a Full-time Infusion Care Coordinator (Patient Ambassador) to support our specialty infusion pharmacy in Woodridge, IL. The role emphasizes data entry, scheduling coordination, and patient-focused care to ensure smooth operations and positive patient experiences.

The position requires on-site presence, strong communication, and multitasking abilities, with a focus on accurate scheduling and inter-department collaboration for optimal patient outcomes.

Qualifications

• Prior healthcare scheduling experience required.
• Home infusion experience preferred.
• Ability to work on-site and interact directly with patients in a clinical setting.
• Strong communication skills.
• Ability to prioritize and multitask.
• Basic computer skills including Microsoft Excel, Word, Outlook.
• High school diploma or equivalent required.

Responsibilities

• Enter clinical information into the clinical management system for billing.
• Coordinate scheduling activities for the nursing department.
• Serve as the single point of contact for referral activity.
• Provide exceptional customer service to internal and external customers.
• Notify patients of financial responsibility and benefit coverage.
• Plan and coordinate nursing visits and ensure timely care.
• Engage with patients after AIS visits to schedule next steps.
