Concordia Community Support Services, part of Concordia Lutheran Ministries, is seeking a dedicated Clinical Informatics Specialist to join our team. We serve communities across Pennsylvania, Ohio, and Florida, and are committed to quality care and organizational excellence.

About the Role

The Clinical Informatics Specialist will be responsible for participating in the planning, development, training, implementation, evaluation, and maintenance of the Concordia Home Care Home Base (HCHB) Electronic Medical Records (EMR). This role requires in-depth knowledge of EMR systems, process optimization, and support for software systems.

Qualifications

• High school diploma or equivalent required; associate or bachelor’s degree recommended or relevant experience
• 1+ year experience in hospice or home health preferred
• Experience with clinical software systems and project management
• Strong organizational, communication, and interpersonal skills
• Reliable transportation with valid driver’s license and insurance
