Qualifications

• Minimum of a High School Education.
• Associates Degree / Bachelor’s Degree preferred.
• Experience working with families of various backgrounds is desirable.
• Familiarity navigating community resources is preferred.

Responsibilities

• Monitors patients’ progress via level of functioning and treatment adherence.
• Schedules outpatient visits and follow-up as needed.
• Provides education and counseling using culturally appropriate strategies; coordinates with Interpreter Services.
• Develops and maintains relationships with community resources to ensure patient access.
• Collaborates with health care team to promote prevention and harm reduction.
• Verifies eligibility and benefits with insurance providers prior to initial consult.
• Documents encounters and maintains records in the electronic medical record (EPIC).
• Performs general administrative tasks and supports Operations Manager.
