Job summary

Concord Hospital in Concord, NH seeks an Infectious Disease RN to provide specialty care under provider supervision and the guidance of the practice manager. You will evaluate and assess patients across infectious disease nursing and HIV populations, coordinating with inpatient, Emergency/ER and Ambulatory Care Center teams.

The role includes case management for patients without resources, patient education, and medication administration, with cross-over to assessment and care planning as needed

Qualifications

• Graduate from an accredited nursing program; Bachelor's degree desirable.
• Certification in Infection Control within 3 years of hire; Licensure as RN in NH.

Responsibilities

• Bases care management actions by applying clinical judgment and sound decisions.
• Interfaces with patients, physicians, nurses, social workers and other departments to coordinate care.
• Manages an independent caseload of patient visits and documents in the EMR.
• Provides patient education and administers prescribed medications.
• Acts as a resource to staff on clinical questions and decisions.
