GW MFA in Washington, DC seeks a Nurse Navigator to educate patients about illness and treatment plans and to coordinate care for Infectious Diseases patients. This role provides in-clinic support to clinicians and works with ID teams and referrals.

The Nurse Navigator will handle urgent orders, help with pre-authorizations, track results, and maintain accurate records. A DC RN license and 2+ years RN experience are required, with a focus on patient-centered care.

Qualifications

• Bachelor's degree in Nursing or higher.
• Current DC Licensure as a Registered Nurse or evidence of application for licensure in DC.

Responsabilités

• Liaison for ID patients requiring coordination of care and support.
• Work collaboratively with admin and clinical ID teams to manage clinical issues, urgent tasks, prescription pre-authorizations and refills, appointments.
• Coordinate and manage all urgent orders for dynamic testing and procedure referrals, including patient preparation and education.
• Follow up with patients on testing and procedure results in coordination with providers.
• Provide clinical support, counseling, and education; alert provider to urgent situations.
• Prepare clinic prep notes for new and returning patients, including scanning notes/imaging and contacting referring physicians.
• Prepare records needed and reconciled for future appointments.
• Assist providers in addressing gaps in care by coordinating follow-up, orders, and referrals.
• Participate in quality improvement, patient safety, and patient experience initiatives.
• Measure and track departmental progress in meeting quality, safety, and patient experience goals.
• Participate in training and education of support staff.
• Maintain up-to-date records/data for programs and patients within MFA and GWUH.
• Identify and coordinate health maintenance issues.
