Lowell Community Health Center, Inc. seeks a Regular Full Time Case Manager to provide comprehensive nursing care for patients with infectious diseases, including HIV, hepatitis, STIs, and other acute and chronic infections.

You will coordinate care with an interdisciplinary team and educate patients to promote treatment adherence and health outcomes. The role emphasizes PrEP/PEP education, ART adherence support, and prevention strategies, within a patient-centered, culturally competent setting.

Qualificações

• Graduate of an accredited school of nursing.
• Current RN license in Massachusetts.
• Minimum of three years of clinical nursing experience.
• Experience in infectious disease, HIV care, public health, primary care, or community health.
• Experience with electronic health records.

Responsabilidades

• Perform nursing assessments and document findings in the EHR.
• Obtain vital signs and medical histories.
• Administer medications, injections, immunizations, and treatments.
• Monitor patients for adverse reactions and changes in condition.
• Coordinate care with providers, nurses, pharmacists, and community agencies.
• Educate patients on HIV prevention, PrEP/PEP, and infection prevention.
• Maintain HIPAA-compliant documentation and infection prevention standards.
