Job summary

Greater Lawrence Family Health Center, Inc. (GLFHC) is seeking an RN Care Manager to join our team.

The role focuses on coordinating care for patients with HIV, viral Hepatitis, and Substance Use Disorder, organizing care delivery, and providing nurse care services such as follow-up visits and education. Responsibilities include intake, care planning, education, scheduling, EMR documentation, and outreach to improve adherence and access to care.

Qualifications

• Massachusetts RN license required.
• Current BLS certification required.
• Minimum of 2 years' experience in case management or care coordination preferred.
• Knowledge of clinical and cultural issues in care of Latino, African American and LGBTQ populations; substance use.
• Strong clinical and assessment skills.
• Bilingual Spanish/English strongly preferred.
• Excellent communication, organization, flexibility and accountability; reliable transportation; computer skills.
• Valid Massachusetts driver's license and access to reliable transportation.

Responsibilities

• Conduct intake and develop culturally competent plans of care to meet health goals.
• Provide education on HIV and viral Hepatitis; counseling on harm reduction and adherence support.
• Lead HIV and Viral Hepatitis clinics by coordinating visits, scheduling patients and managing clinic flow.
• Document contacts in EMR per protocol.
• Coordinate care with PCP, HIV/HCV specialists, CHW, nutritionist, pharmacist and team.
• Coordinate outreach to new patients and assess progress toward goals.
