Job summary

Community Health Center of Southeast Kansas (CHC/SEK) seeks an RN – Infectious Disease Care Coordinator to join a multidisciplinary team. The role provides comprehensive nursing care, care coordination for infectious disease services (HIV/Hepatitis C), MAT, and related duties to support safe, patient-centered care.

Responsibilities include in-clinic care, triage, education, and acting as a primary link between patients and providers.

Qualifications

• Must hold a Kansas RN license and be eligible for multistate licensure.
• 1 year nursing experience in infectious disease, MAT, or ATS preferred.
• BLS certification required within 90 days of hire.
• Training for HIV, Hepatitis C, and SBIRT within 90 days as assigned.

Responsibilities

• Oversee patient rooming, collect histories, verify medications and vital signs.
• Support clinicians with procedures, injections, phlebotomy, EKGs, and in-house labs.
• Supervise vaccine administration accuracy and safety per protocols.
• Assist with medication refills, prior authorizations, and documentation.
• Coordinate triage, labs, specialty services, and daily care team huddles.
