Job summary

Ohio State University Physicians is seeking a proactive coordinator to bridge physician offices, clinics, and patients across the care continuum. You will manage patient data, schedule workflows, and support referral processes within a collaborative outpatient environment.

Daily tasks include coordinating appointments, documenting care plans, and assisting with access to community resources. A strong customer-service mindset and confidentiality are essential to patient-centered care.

Qualifications

• High School diploma or equivalent; one year of customer service or medical experience; or equivalent combination of education and experience.
• Familiar with guidelines regarding patient confidentiality issues.
• Must have working knowledge of MS Office platforms such as Excel, OneNote, Word, PowerPoint.
• Ability to convey information both in writing and verbally.
• Must have high level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service.

Responsibilities

• Works as the coordinator between physician offices, specialty clinics, hospital departments and patients to promptly meet the needs of patients scheduled for clinic appointments and procedures as well as referrals across the organization.
• Maintains the assigned minimum number of patient cases longitudinally and tracks large data such as patient profiles, appointments and interactions.
• Coordinates multidisciplinary planning conferences and documents recommendations using standardized care protocols.
• Collaborates to assess gaps in patient outcomes and supports continuous quality improvement with data sharing to management.
• Contacts patients prior to appointments to confirm and resolve barriers; prepares care team and enters test orders/insurance authorizations.
• Assists patients and families by identifying resources for medical, insurance, or social needs; provides referrals to community agencies.
• Follows up with patients throughout care plans and may escort patients as needed to pre/post procedure areas.
• Identifies opportunities for improvement in patient care and organizational processes.
• Anticipates customer needs and resolves issues; contributes to performance improvement.
