Job summary

Ohio State University Physicians seeks a Clinical Nurse Care Coordinator to partner with clinic teams, delivering patient-centered care and ensuring care plans remain current and comprehensive.

You will work with high-risk and medically complex patients, including those on disease registries or in care transitions, coordinating communication among families and care partners to achieve better health outcomes.

Qualifications

• Associate Degree in Nursing from a CCNE or ACEN accredited program.
• Current Ohio RN license.
• CPR/BLS certification.
• Knowledge of triage procedures with clinic/urgent care experience.
• Ability to maintain quality, safety, and infection control standards.
• Knowledge of medication administration and patient care aids.
• Ability to educate and clearly communicate with patients and families about disease and care.
• Strong written and verbal communication; ability to convey information clearly.
• High interpersonal skills to handle sensitive/confidential situations.
• Poise, tact and diplomacy; analytical thinking to interpret data and solve problems.

Responsibilities

• Collaborate with clinic team to provide evidence-based, patient-centered care and keep care plans up to date.
• Focus on high-risk medically complex patients and those on disease registries or in care transitions.
• Serve as liaison with patients' families and other agencies to improve communication and care.
• Stay current on standards and assist with programs and processes.
