Job summary

Qualifications

• Completion of required certifications or continuing education in community health within the first year.
• High School Diploma or GED is required.
• 1–2 years of relevant experience.
• Ability to engage diverse populations and collaborate with multiple partners.
• Knowledge of social drivers of health and health literacy principles.

Responsibilities

• Provides direct services to clients, including health education and prevention programs.
• Maintains confidential client records and ensures accuracy.
• Acts as liaison between clients and healthcare providers.
• Conducts follow-ups to ensure client needs are met.
• Conducts home visits or community events to reach populations.
• Educates on self-management of health conditions and proper use of emergency services.
