Qualifications

Responsibilities

• Demonstrate knowledge of infant-toddler development.
• Utilize a parent coaching model with families.
• Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as part of a trans-disciplinary team.
• Provide home-based services to families across the catchment area.
• Collaborate with community providers to ensure continuity of services for families including child care, medical, social services, and specialist providers.
• Provide service coordination to assigned caseload including development, monitoring, and review of the Individual Family Service Plan.
