Job summary

Hsmc1 is seeking a Pediatric Physical Therapist to provide home-based services and participate in trans-disciplinary care teams across the Greenfield area. You will contribute to infant-toddler development initiatives, coach families, and coordinate services with community providers.

The role requires MA PT licensure, pediatric experience, strong communication skills, and proficiency with Microsoft Office and related tools. A valid driver's license and reliable transportation are required.

Qualifications

  • Current licensure as a Physical Therapist by the Massachusetts Board of Registration in Allied Professional Health.
  • Two years of pediatric experience preferred.
  • Effective verbal and written communication skills.
  • Valid driver's license and reliable transportation.
  • Working knowledge of Word, Excel, Adobe, and Office365 including Outlook, OneDrive, and Teams.

Responsibilities

  • Demonstrate knowledge and understanding of all areas of infant-toddler development.
  • Utilize a parent coaching model in work with families.
  • Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as a member of a trans-disciplinary team.
  • Provide home-based services to families across the catchment area.
  • Collaborate with community providers to ensure continuity of services for families including child care providers, medical providers, social service agencies, and specialty providers serving children with autism or hearing/vision loss.
  • Provide service coordination to assigned caseload including coordinating the development, monitoring, and review of the Individual Family Service Plan.
  • Current licensure as a Physical Therapist by the Massachusetts Board of Registration in Allied Professional Health.
  • Two (2) years of pediatric experience preferred.
  • Effective verbal and written communication skills.
  • Working knowledge of Word, Excel, Adobe, and Office365 including Outlook, OneDrive, and Teams.
  • Ability to use computer-based technology including a computer/laptop and cell phone.
  • Valid driver's license and reliable transportation is required.
