Job summary

PALS OHIO, LLC is seeking an Individual Support Coordinator to collaborate with the interdisciplinary team and oversee supports for individuals on their caseload. You will coordinate transportation, document attendance, incident reports, and ensure program activities are meaningful and compliant with policies.

You will maintain relationships with community partners, participate in staff meetings, and support staff with person-first language and respectful interactions.

Qualifications

• Must have a current and valid Driver’s License.
• Criminal Background verification with acceptable clearance.
• Willingness to work flexible schedule and provide responsible supervision.
• Must pass DOT physical.

Responsibilities

• Oversee coordination of transportation needs with Transportation Coordinator and Program Director.
• Collect daily attendance and report changes to the Program Director.
• Ensure individuals receive meaningful programming and that documentation reflects daily activities.
• Review and ensure timely completion of all program documentation including transportation and billing.
• Collect, review, and submit unusual incident reports with appropriate action and notification.
• Contribute to weekly staff meetings and support a positive, respectful environment.
• Build and maintain professional relationships with community partners for program planning.
