Job summary

FeldCare Connects in Torrance seeks a self-motivated Physical Therapist to deliver premier home health care to adults of all ages. This Independent Contractor role focuses on assessment, planning, and in-home treatment to promote movement, reduce pain, restore function, and prevent disability.

You will determine and execute a personalized treatment plan, monitor progress, and communicate clearly with patients and care teams.

Qualifications

• Experience delivering in-home physical therapy to adults.
• Current state PT license and registration.
• Ability to work with patients of all ages and environments.
• Bilingual skills are a plus.

Responsibilities

• Assess and evaluate home health patients to determine care needs.
• Develop and implement a treatment plan to improve mobility and reduce pain.
• Monitor progress and adjust interventions as patient responds.
• Document care and maintain records with accuracy.
