Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist Assistant to deliver premier home health care in the Anaheim area. This Independent Contractor role focuses on assessing patient needs, planning treatment, and supporting movement, pain reduction, and functional restoration for adults.

The ideal candidate will hold a State license, have a PTA credential, and be comfortable delivering individualized care with flexibility, independence, and strong communication with patients and care

Qualifications

• Ability to assess and evaluate patient care needs in home health settings.
• Strong communication and documentation to support care plans.
• Ability to adapt treatment plans based on patient progress.

Responsibilities

• Assess patient needs and develop treatment plans for PTAs in home health.
• Deliver evidence-based physical therapy interventions to improve function and reduce pain.
• Monitor patient progress and adjust care plans in collaboration with supervising therapists.
