Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier in-home care for adults in the South Gate area. As an independent contractor, you will assess, plan, and implement therapy to improve mobility and reduce pain in daily living activities.

You will tailor treatment, document progress, and communicate clearly with patients and caregivers while maintaining confidentiality and professional standards. A state license and eligible PT curriculum are required.

Qualifications

• Licensed Physical Therapist in the state.
• Completed PT curriculum approved by APTA CMA or related council.

Responsibilities

• Assess and evaluate patient care needs.
• Develop and execute a treatment plan to improve movement and function.
• Monitor patient progress and adjust interventions as needed.
• Promote movement, reduce pain, restore function and prevent disability.
