Qualifications

• Current Physical Therapy license issued by the state board; active and unrestricted.
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation to patient homes.
• Smartphone required for app-based coordination.
• Bilingual abilities are a plus.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe and report patient response to treatment and adjust plan of care.
• Coordinate care using Cliniconnects app to accept or decline cases.
