Job summary

FeldCare Connects is seeking an enthusiastic Physical Therapist to provide quality home healthcare in Elk Grove and surrounding areas as an independent contractor. You will manage one-on-one patient treatment, observe responses, and report changes while maintaining an accurate EMR documentation process.

The role emphasizes flexible scheduling and autonomous practice as you participate in delivering patient-centered care across ages.

Qualifications

• Ability and enthusiasm in working with all ages.
• Current Physical Therapist license issued by the state board of Physical Therapy.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Bilingual a plus.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care with one-on-one treatment.
• Utilize an Electronic Medical Record system for documentation during visits.
