FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver quality home healthcare as an independent contractor serving Dinuba and surrounding areas. You control your own schedule and coverage, and you earn based on per-visit compensation while receiving administrative support and a one-on-one treatment approach.

Cliniconnects provides streamlined referrals and an intuitive EMR system to simplify documentation, enabling you to focus on patient care and outcomes.

Qualifications

• Provides home health PT to patients of all ages.
• Independent contractor; control your own schedule and coverage area.
• Current PT license issued by the state; unrestricted license required.
• Own smartphone to document and coordinate care.

Responsibilities

• Observe, record, and report patient responses to treatment and changes in condition.
• Determine the plan of care in collaboration with patients and caregivers.
• Provide one-on-one treatment in patients' homes.
• Document visits efficiently using the EMR during or immediately after treatment.
• Utilize Cliniconnects app to accept or decline patients quickly.
