Job summary

FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver quality home health care to patients of all ages in Chico and surrounding areas. This Independent Contractor role requires a current PT license in the state and completion of an accredited PT program.

You will have flexible scheduling, operate as your own boss, and determine earnings through per-visit rates. The company provides administrative support and access to Cliniconnects for streamlined referrals and documentation.

Qualifications

• Current Physical Therapist license issued by the state Board of Physical Therapy.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation and a smartphone.
• Bilingual a plus.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine and implement the plan of care.
• Complete documentation efficiently in the EMR during visits.
• Coordinate with administrative support to ensure smooth care delivery.
