FeldCare Connects is seeking a self-motivated Physical Therapist for home health in the Rocklin area. This Independent Contractor role focuses on delivering premier care to adults in their homes.

The PT assesses needs, develops treatment plans, implements interventions, and tracks progress to promote movement, reduce pain, and restore function while maintaining accurate records and demonstrations of clinical reasoning.

Qualifications

• Ability and enthusiasm in working with all ages
• Physical Therapist license and state registration
• Curriculum approved by APTA or related bodies
• Bilingual a plus

Responsibilities

• Assess and evaluate patient care needs in home health
