Walmart's Sam's Club in Bristol, VA is seeking an Independent Optometrist leasing space inside our stores to lead eye and vision care as well as general health and wellbeing.

You will establish your days of practice, hours of operation, patient schedule and fee schedule, keeping 100% of exam/professional fees. Optional advanced equipment and support services may be available where permitted by state regulations.

Qualifications

• OD degree from an accredited optometry school
• Valid state optometry license required
• Ophthalmology MD/DO from an accredited school
• Valid state ophthalmology license required
• General Liability and Professional Liability Insurance Certificates
