Qualifications

• OD degree from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD/DO with valid state ophthalmology license.

Responsibilities

• Launch optometry services in Sam's Club locations.
• Establish days, hours, and patient schedule.
• Set fee schedule and manage exam/professional fees.

Education
