You will establish your practice days, hours, and patient schedule, set your fee schedule, and keep 100% of your exam fees.

Qualifications

• OD from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist MD or DO from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate/Professional Liability Insurance Certificate.
• ME/VA license agreements are with Gumberg Asset Management Company.

Responsibilities

• Launch your optometry services in Sam's Club.
• Lead patient care in eye/vision health and wellbeing.
• Establish your days of practice and hours of operation.
• Control your patient schedule and set your fee schedule.
• Keep 100% of exam/professional fees.
• Opt into advanced equipment/instrumentation and admin/Opt-Tech support if state allows.

Doctor of Optometry (OD) from an accredited optometry school

Ophthalmologist MD/DO from an accredited school
