Job summary

Gilder Search Group seeks a Nurse Practitioner to perform in-home Annual Wellness Visits for Medicare members, focusing on prevention and education with no prescribing. You’ll travel within a 30–50 mile radius, schedule your own hours, and operate as a 1099 contractor with flexible full- or part-time options.

Responsibilities include conducting comprehensive wellness assessments, educating members on benefits, and documenting in EMR systems, while receiving malpractice insurance and mileage

Qualifications

• Active State NP license and Board Certification.
• Valid NPI number and ability to travel 30–50 miles.
• Experience in wellness exams and preventive care.

Responsibilities

• Conduct comprehensive in-home wellness assessments.
• Educate members on preventive care benefits.
• Identify gaps in care to reduce avoidable ER visits.
• Complete documentation using EMR tools.
• Face-to-face visits at members' homes.
