Job summary

Gilder Search Group is seeking a Wellness Visit Nurse Practitioner to conduct in-home wellness assessments across Sacramento, CA and surrounding areas. This flexible 1099 contract role offers full-time or part-time hours, with travel within a 30–50 mile radius and no prescribing responsibilities.

You’ll educate members on preventive care, identify gaps in care to reduce avoidable ER visits, and complete documentation via EMR tools using provided devices.

Qualifications

• Active NP license and board certification required.
• Experience conducting in-home wellness exams preferred (minimum 3 months).
• Comfort traveling within a 30–50 mile radius and working 1099/contract terms.

Responsibilities
