Qualifications

• Active RN and APRN license(s) in state of practice.
• National board certification in area of clinical practice and populations served.
• Active DEA registration prior to hire.

Responsibilities

• Assess, diagnose, and determine treatment plans per patient age and condition.
• Manage conditions based on evidence-based care and risk/benefit assessment.
• Provide health promotion and disease prevention counseling to patients and families.
• Collaborate with an interdisciplinary health care team and refer as needed.
• Maintain accurate documentation in the electronic medical record per coding and billing rules.
• Facilitate coordinated care and clear communication among health care team and community agencies.
• Perform office or hospital procedures as allowed by privileges.
