Job summary

Residential Living Services is seeking a Direct Care Professional to assist individuals with daily living activities and promote independence. The role involves teaching decision-making, documenting progress, and ensuring safety in the home environment.

Responsibilities include medication administration, meal planning, transportation, and coordination with the Case Manager. Must have a High School Diploma or GED, be at least 18, and hold a valid driver’s license.

Qualifications

• Requires High School Diploma or GED and ability to communicate effectively.
• Needs to pass a Criminal Record Check and have a valid driver’s license.
• Ability to provide physical assistance and monitor safety according to plan.

Responsibilities

• Provide direct support to consumers in daily living activities.
• Assist with ambulation, dressing, grooming, eating, toileting and meal preparation.
• Administer medications and monitor adherence with proper labeling and documentation.
• Transport consumers to activities and appointments in a safe manner.
• Document daily progress and communicate with the Field Coordinator regularly.
• Attend required in-service trainings and maintain confidentiality and HIPAA compliance.
