Qualifications

Responsibilities

• Provide direct care and assist consumers with daily living activities and personal hygiene.
• Support consumers in achieving independence and following Personal Support/Habilitation Plans.
• Assist with meal planning, preparation, and shopping; manage household tasks and budgeting.
• Coordinate with Case Manager and medical staff; accompany to appointments as needed.
• Ensure safety, confidentiality, and HIPAA compliance; document incidents and activities.
