Job summary

UCP of Central Arizona is seeking staff to assist members with independent living tasks, support living skills, and provide respite and care services at the Laura Dozer Main Campus in Phoenix, AZ. The role focuses on documenting progress, maintaining privacy, and collaborating with families and care teams.

Responsibilities include scheduling, billing, reporting, and ensuring services align with individual support plans, while fostering positive relationships and adhering to HIPAA.

Qualifications

• Must be 18 years or older.
• Background check and drug screening as part of pre-employment.
• Must have vehicle to transport member when approved by family and ISP.
• Must have internet access to communicate with UCP (email, timesheets).
• Must have general computer skills to input data into DDD software and other UCP programs.

Responsibilities

• Schedules services with member and family to meet needs.
• Prepares billing and reporting documents within required timelines.
• Ensures service hours are implemented according to ISP and shift calendar.
• Documents member progress daily for habilitation and attendant care goals.
• Maintains confidentiality of member information per HIPAA policies.
• Fosters positive relationships with members, families, other providers and external partners.
• Participates in ISP meetings and attends required trainings.
