Job summary

Victorian Senior Care seeks a compassionate direct support professional to work one-on-one with a person with intellectual/developmental disabilities (I/DD) to help them reach individualized goals and maximize independence. Positions may be in private homes or community settings in North Carolina.

As a direct support professional, you will implement daily living plans, document progress daily, and accompany the person to appointments and activities.

Qualifications

• Must be at least 21 years old.
• High School Diploma or GED.
• Must be able to read, write, understand, and follow directions.
• Must have no substantiated findings on the NC Health Care Abuse Registry.
• Must meet SBI and DMV requirements of LME/MCO contracts.
• Must have a valid driver’s license and 3 full years of driving experience.
• Must have acceptable automobile liability insurance.
• Experience with persons with intellectual/developmental disabilities preferred but not required.
• Must be willing to complete all required trainings as assigned.
• Must have access to a device with internet connection at the time services are rendered.

Responsibilities

• Implement individual habitation plan involving daily living skills, socialization, employment, leisure and/or recreational activities.
• Document the service provided in progress notes daily with observations and progress.
• Obtain guardian’s initials/signatures when required on documentation.
• Provide transportation for the person supported to community services/activities when required.
• Assist person supported with money management.
• Encourage person supported to make decisions on his own to maximize independence.
• Assist with prescribed exercises taught by a professional.
• Perform incidental household services essential to care at home.
• Monitor progress notes, charts, and programming materials; maintain records.
• Submit all required documentation by deadlines; complete mandatory trainings (min 12 hours/year).
• Be familiar with agency policies and emergency services regulations including medication administration.
• Attend mandatory staff meetings; report changes to Service Coordinator/Supervisor.
