Job summary

Chimes is seeking a dedicated direct support professional to provide personal support in residents’ homes, ensuring activities align with individual plans and goals.

You will supervise adults with developmental disabilities, promote independence, and maintain a safe, personalized living environment while coordinating with program staff and health professionals. Benefits reflect a comprehensive, mission-driven organization.

Qualifications

• High School diploma or equivalent preferred.
• Must have a driver’s license and an acceptable driving record.
• Ability to lift and push up to 50 pounds.

Responsibilities

• Provide personal support in the residence and engage the person served.
• Supervise and train adults with developmental disabilities per individual plans.
• Ensure the house is personalized to each person’s preferences.
• Teach activities to foster independence and limit personal cell phone use during work.
• Communicate with day program staff about the individual’s program.
• Monitor safety and house operations; report concerns promptly.
• Apply approved behavior plans and interventions to aid progress.
• Act as a positive role model and work hands-on with residents.
• Assist with daily living skills, mobility, budgeting, and hygiene.
• Transport residents to appointments and community activities.
• Manage funds per agency policy and report financial activity.
• Communicate medical concerns to the RN and supervisor.
• Perform routine checks on rooms for safety and wellbeing.
• Attend meetings and participate in staff training and teamwork.
• Adhere to agency policies and assume additional duties as needed.
