Job summary

Public Partnerships seeks consultants (Support Brokers) to provide supports brokerage under contract with the California Department of Human Services. You will help participants exercise choice and control over community services, enroll direct care workers, and monitor health and safety while maintaining progress notes.

Qualifications

• BA or BS degree in related field required, can be substituted for 4 or more years of related experience.
• Minimum of two (2) year of experience working with individuals with disabilities and/or senior citizens required; knowledge of Medicaid services preferred; bilingual in English and Spanish preferred.

Responsibilities

• Manage Workload.
• Maintain required contact with assigned participants, monthly telephone calls.
• Respond to participant inquiries.
• Maintain email communication with participants, as needed.
• Resolve participant issues.
