Job summary

SPM, LLC – Branch Creek Apartments in Athens, AL, seeks a Service Coordinator to connect residents with supportive services, assess needs, and develop plans that promote independence and safe housing. You’ll coordinate wellness programs, maintain documentation, and collaborate with local providers and management to deliver tailored resources.

Required is a Bachelor’s degree in Social Work, Gerontology, Psychology, or related field, plus strong communication, organization, and problem-solving

Qualifications

• Bachelor’s degree required in Social Work, Gerontology, Psychology or related field.
• Strong case management and problem-solving skills with a team-oriented attitude.
• Excellent communication and relationship-building skills.

Responsibilities

• Empower residents to live independently by connecting them with essential services, community resources, healthcare providers, and social service agencies.
• Build and maintain a robust network of local agencies, providers, and wellness resources.
• Educate and guide residents and families through Federal, State, and local programs tailored to senior adults.
• Assist residents in accessing benefits such as Medicare, Medicaid, SNAP, SSI, transportation, and other assistance programs.
• Coordinate wellness and educational programs that promote healthy aging and independent living.
• Conduct regular follow-up with residents to ensure services remain appropriate and effective.
• Maintain accurate resident files and documentation in compliance with HUD and company requirements.
• Complete assessments, referrals, case notes, and reports in a timely manner.
• Respond to resident concerns and assist with problem-solving.
• Build relationships with local service providers to expand available resources for residents.
• Collaborate with the management team to identify residents in need and provide proactive assistance.
• Advocate and negotiate on behalf of residents for cost-effective and adequate services.
• Create and maintain an up-to-date directory of community resources for residents and staff.
