Job summary

SPM, LLC is seeking a Service Coordinator for Branch Creek Apartments in Athens, AL. The role focuses on linking residents with services to promote independence and safe housing, with assessments, service plans, and referrals to community resources.

Ideal candidates have a Bachelor's in Social Work, Gerontology, or Psychology, plus strong case management, communication, and initiative. Compensation is $19–$21 per hour for daytime hours across 3 days per week.

Qualifications

• Bachelor's in Social Work, Gerontology, Psychology, or related field.
• Knowledge of local services, state and federal programs, and senior resources.
• Understanding of Medicare, Medicaid and other public benefit programs for seniors.
• Strong case management and problem-solving skills with a team-oriented attitude.
• Excellent communication and relationship-building skills
• Organized, resourceful, and ready to take initiative

Responsibilities

• Link residents with supportive services to promote independence and improve quality of life.
• Conduct resident assessments and develop service plans.
• Coordinate educational and wellness programs; provide referrals to community resources.
• Maintain documentation and ensure HUD/compliance with company policies.
• Collaborate with property management and local service providers; follow up on services.
• Advocate and negotiate for cost-effective, adequate services for residents.
• Create and maintain an up-to-date directory of community resources.
