Job summary

SPM, LLC in Birmingham, AL is seeking a Service Coordinator to link residents with supportive services that promote independence and safe housing. You will assess residents, develop service plans, and coordinate programs while aligning with HUD regulations and company policies.

This role requires a Bachelor's degree in Social Work or related field, strong case management and communication skills, and a passion for empowering seniors.

Qualifications

• Bachelor’s degree in Social Work, Gerontology, Psychology, or related field (Required)
• Knowledge of local services, state and federal programs, and senior resources.
• Understanding of Medicare, Medicaid, Social Security and other public benefit programs for seniors.
• Strong case management and problem-solving skills with a team-oriented attitude.
• Excellent communication and relationship-building skills
• Organized, resourceful, and ready to take initiative

Responsibilities

• Empower residents by connecting them with essential services and resources.
• Build and maintain a network of local agencies and wellness resources.
• Guide residents through federal, state, and local programs for seniors.
• Assist residents in accessing benefits such as Medicare, Medicaid, SNAP, SSI, transportation.
• Coordinate wellness and educational programs promoting healthy aging and independence.
• Maintain resident files in HUD and company-compliant documentation.
• Complete assessments, referrals, case notes, and reports timely.
• Respond to resident concerns and assist with problem-solving.
• Collaborate with management to identify residents in need and provide proactive assistance.
