Job summary

Envision Unlimited is seeking a Qualified Intellectual Disabilities Professional (QIDP) to coordinate comprehensive case management in community living settings. You will implement individualized programs, document outcomes, and liaise with guardians, staff, healthcare providers, and stakeholders to ensure compliance with state and agency requirements.

Qualifications

• Bachelor's degree in Human Service field required.
• Must be a QIDP; verification letter is required.
• Excellent written and communication skills; computer and organizational skills.
• Proof of Direct Service Professional training and ability to pass STOFHLA test.

Responsibilities

• Develop and implement individualized, progressive programs for members with intellectual and/or developmental disabilities in community living settings.
• Schedule, assign, document, and monitor member activities to ensure goals and outcomes are implemented.
• Develop Implementation Plans and complete required assessments, including ICAP and SLOF.
• Ensure accurate and timely completion of CIRAS, incident, and hospitalization reports in the electronic health record system.
• Submit required documentation to Human Rights Committee and Behavior Management Committee and attend meetings as required.
• Coordinate professional and consultative services and ensure implementation of recommended services.
• Attend member psychiatric and medical appointments as needed.
• Maintain current, accurate member records and attendance reports in compliance with guidelines.
