Job summary

BRC is seeking a dedicated Case Manager to provide comprehensive services for mentally ill, homeless adults. The role includes benefits assistance, treatment referrals, hygiene monitoring, and independent living skills coaching.

You will lead weekly psychoeducational groups, document progress, and prepare monthly reports. Collaboration with clinical teams and staff meetings is expected, with CPR support as needed.

Qualifications

• High School Diploma/GED required; bachelors degree preferred and masters ideal.
• Bilingual Spanish strongly preferred, but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience working in residential treatment and with the MICA and homeless populations.
• Knowledge of HRA and NY/NY application systems a plus.
• CPR training certification or willingness to take training class in CPR.

Responsibilities

• Provide comprehensive case management services to a challenging caseload of mentally ill, homeless adults.
• Assist with obtaining benefits, referrals for treatment, monitoring hygiene and teaching independent living skills.
• Escorts to and advocacy for entitlements and housing appointments, and housing placement.
• Facilitate weekly psycho educational group.
• Generate ongoing progress and group notes as well as monthly statistical reports.
• Participate in clinical team, case conference, and other staff meetings.
• Assist with program staff coverage as needed.
• Provide emergency first aid/CPR assistance when needed.
• Perform additional related duties as assigned.
