Job summary

BRC in New York City seeks a dedicated housing program case manager to support clients in community residences, meeting at least weekly.

You will make referrals to medical, mental health, and substance abuse services, provide supportive counseling, teach daily living skills, and monitor safety.

You will complete housing applications, service plans, psychosocial reports, assist with intake, participate in community meetings, and document care thoroughly.

Qualifications

• BA preferred; HS diploma/GED required and significant experience working with the substance abuse and/or mentally ill population.
• Excellent writing skills.
• Basic computer literacy.
• CPR training or willingness to take training class in CPR.
• Vaccination preferred but not required.

Responsibilities

• Manage assigned caseload, meeting clients weekly.
• Refer to medical, mental health and/or substance abuse services and coordinate care.
• Provide clients with supportive counseling.
• Visit client rooms, monitor safety, and assist with activities of daily living.
• Complete client housing applications, service plans, and psychosocial reports.
