Job summary

Trinity Services, Inc. is seeking an Independent Living Coach (ILC) to support adults with intellectual disabilities and/or mental illness in home and community settings. No prior experience is required; training is provided to develop life skills and independence.

Qualifications

A valid High School Diploma or GED from a recognized school is required.
A valid driver’s license and motor vehicle report with minimal traffic violations.
Pass a STOFLA assessment (reading literacy) prior to first day of employment.
Will be trained and required to maintain active CPR, First Aid, and Safety Care certifications as well as Medication Administration Authorization.

Responsibilities

Foster relationships with people receiving services to support independence.
Provide daily living support with planning meaningful activities.
Assist with meal prep and follow dietary needs.
Provide transportation in an agency vehicle to appointments and activities.
Document activities and report medical/behavior changes to the nurse and supervisor.
Follow written instructions from plans and orders and perform other duties as assigned.
