Job summary

Achieve Behavior Alliance Plus, LLC is seeking Home & Community Support Professionals (DSPs) to provide person-centered, direct care to individuals with intellectual and developmental disabilities in home and community settings in Indiana. Contractors offer flexible daytime, evening, and weekend hours under an independent contractor agreement.

Responsibilities include assisting with daily living activities, following personalized care plans, documenting progress, and coordinating with families

Qualifications

• DSP Core Certification (Indiana) and required certifications
• CPR & First Aid certification
• Negative TB test
• Clean background check
• HIPAA course completion

Responsibilities

• Provide person-centered direct care support to individuals with intellectual and developmental disabilities
• Assist with daily living activities and independence-building tasks
• Follow individualized support plans to promote skill development and participation
• Document changes in health, behavior, or well-being
• Complete accurate service documentation and notes
• Coordinate schedules with individuals and families
