Job summary

Achieve Behavior Alliance Plus, LLC seeks a Direct Support Professional to provide person-centered care in Fishers, Indiana, serving participants with intellectual and developmental disabilities. The role includes PAC, Respite, Day Habilitation, and other services under an independent contractor agreement.

Contractors can set schedules, select client locations, and work daytime, evenings, or weekends, with necessary Indiana DSP certification and HIPAA obligations.

Qualifications

• Indiana DSP Core Certification required (Achieve assistance available)
• CPR & First Aid certification
• Negative TB test
• Clean background check
• HIPAA course completion
• Valid driver’s license with reliable transportation

Responsibilities

• Provide person-centered direct care to individuals with developmental disabilities
• Assist with daily living activities and independence-building tasks
• Follow individual support plans to promote skill development
• Maintain a safe, respectful environment and document changes
• Complete accurate service documentation and session notes
• Communicate professionally with families and coordinators
• Coordinate schedules with individuals and families
• Supervise and engage clients during caregiver absences
• Offer meaningful daytime activities and community involvement
• Collaborate with support teams while working independently
