Job summary

Achieve Behavior Alliance Plus, LLC seeks Direct Support Professionals to provide person-centered in-home and community-based support in Indianapolis, Indiana. Contractors assist individuals with intellectual and developmental disabilities through PAC, Respite, Day Habilitation, and Career Exploration services, following individualized care plans.

As a contractor, you set your schedule, work independently, and collaborate with a support team as needed, with evening hours and weekends preferred.

Qualifications

• Must be 18 years of age or older.
• High school diploma or equivalent preferred, not required.
• Valid driver's license with reliable transportation to and from clients.
• Clean background check.
• HIPAA Course Completion.
• Experience working with individuals with developmental, intellectual disabilities, or autism.
• Ability to work independently and with a team.

Responsibilities

• Provide person-centered, direct care support to individuals with intellectual and developmental disabilities.
• Assist with daily living activities and independence-building tasks.
• Follow each individual’s support plan to promote skill development and community participation.
• Maintain a safe, respectful environment while protecting dignity and privacy.
• Observe, document, and report changes in health or well-being.
• Complete accurate service documentation and session notes.
• Communicate professionally with families, coordinators, and other providers.
• Coordinate schedules with individuals and families.
• Supervise and engage individuals during caregiver absences (service-specific).
• Facilitate social, recreational, and skill-building activities in home and community settings.
