Job summary

Achieve Behavior Alliance Plus, LLC is seeking Home & Community Specialist Professionals in Carmel, Indiana, to provide person-centered support in in-home and community settings. Contractors manage their own client load, schedule, and services, delivering direct support across PAC, respite, day habilitation, and employment exploration.

The role emphasizes independence, meaningful community participation, and collaboration with a team when needed; prior DSP experience and relevant certifications

Qualifications

• Must be 18 years of age or older and able to meet client needs.
• High school diploma or equivalent preferred; professional credentials help.
• Valid driver’s license and reliable transportation to client locations.
• Active certifications (DSP, CPR/First Aid) and background checks current.
• Ability to obtain required certifications at contract start.

Responsibilities

• Provide person-centered, direct care support to individuals with disabilities.
• Assist with daily living activities, meal prep, hygiene, mobility and independence-building tasks.
• Follow support plans to promote skill development and community participation.
• Document changes in health or behavior and prepare session notes.
• Coordinate schedules with families and other providers; work independently yet with a team.
• Engage participants in meaningful daytime routines and community activities.
