Job summary

Achieve Behavior Alliance Plus, LLC offers Home & Community Support roles as Independent Contractor positions in the Indianapolis area. DSPs provide in-home and community-based support with flexible scheduling to fit families’ needs.

Responsibilities include daily living assistance, skill-building, and community participation, with emphasis on person-centered care. Contractors determine client load, locations, and hours, aligning work with personal goals and schedule.

Qualifications

• Must be 18+ years old.
• High school diploma or equivalent preferred.
• Valid driver’s license with reliable transportation.
• Clean background check; consent to OIG and SAM checks.
• DSP or caregiving experience with children or adults.
• Familiarity with EVV and digital documentation.

Responsibilities

• Provide person-centered, direct care support to individuals with intellectual and developmental disabilities.
• Assist with daily living activities, including meals, personal hygiene, mobility, and independence-building tasks.
• Follow each individual’s support plan to promote skill development, communication, and community participation.
• Maintain a safe, respectful environment while protecting dignity and privacy.
• Observe, document, and report changes in health or well-being.
• Complete accurate service documentation and session notes.
• Coordinate schedules directly with individuals and families.
• Supervise and engage individuals during caregiver absences (service-specific).
• Facilitate social, recreational, and skill-building activities in home and community settings.
• Support adults with meaningful daytime routines and community inclusion.
• Assist individuals in exploring employment interests and developing job readiness and workplace skills.
