Job summary

FeldCare Connects in San Diego is seeking a self-motivated Speech Therapist to deliver premier home health services for adults. This independent contractor role offers flexibility and autonomy while making a meaningful difference in patients' lives.

You will assess patients, develop and implement treatment plans for speech, language, swallowing, and cognitive needs, and monitor progress. Collaboration with families and the clinical team is essential, with detailed documentation and ongoing

Qualifications

• State license and registration to practice Speech Language Pathology.
• CCC-SLP certification by ASHA.
• Completion of an accredited Speech-Language Pathology program.
• Bilingual abilities are a plus.

Responsibilities

• Assess and evaluate patient care needs for speech and language disorders.
• Develop and implement treatment plans; provide interventions.
• Monitor progress and report patient responses to treatment.
• Educate patients, families, and health team on therapy procedures.
• Document sessions and update medical records.
• Make recommendations regarding continuation or modification of therapy.
