FeldCare Connects in Fairfield, CA is seeking a self-motivated Speech Therapist to deliver high-quality care to adults with speech, language, swallowing, and cognitive needs in home health settings as an Independent Contractor. You will assess, plan, implement, and monitor treatment to improve patient communication and overall well-being.

As part of the role, you will document visits, collaborate with families and health team, and adapt plans to each patient’s needs.

Qualifications

• Ability and enthusiasm in working with all ages.
• State license and registration as a Speech Therapist.
• Completion of an accredited Speech Therapy program.
• CCC-SLP (ASHA) certification.

Responsabilités

• Assess and evaluate patient care needs for speech and language disorders.
• Develop treatment plans and implement interventions to improve communication, swallowing, and cognitive function.
• Monitor patient progress and adjust treatment as needed.
• Document patient visits and update medical records.
• Collaborate with family and health team to optimize patient outcomes.
