Job summary

Adaptive Home Health is seeking a Physical Therapist Assistant (PTA) for home health in Texas. This field-based, full-time role offers a productivity-based pay model and a schedule of Monday–Friday.

You will visit patients in their homes under a supervising PT and focus on mobility, safety, gait, and functional goals. You will be supported by an experienced clinical and operational team, with charting and scheduling tools designed to streamline care and protect your time.

Qualifications

• Active Texas PTA license (in good standing).
• Strong clinical judgment and ability to work independently in the field.
• Comfort escalating changes in condition to PT/RN/MD with clear documentation.
• Strong communication and documentation skills.
• Reliable follow-through on treatment plans and goals.
• Valid driver’s license and willingness to travel daily across territory.
• Ability to carry/transport basic therapy equipment.
• Core PTA scope: gait training, balance, transfers, home safety, and patient education.

Responsibilities

• Deliver skilled PTA visits in the home under the supervising PT’s plan of care.
• Execute treatment plans focusing on functional mobility and progress.
• Set/track objective progress and communicate updates to the supervising PT.
• Drive multidisciplinary coordination to align orders and keep the care team moving.
• Coach patients and caregivers with practical home programs.
• Document promptly in Homecare Homebase (HCHB) with defensible records.
