Job summary

VitalCaring is seeking a Home Health Physical Therapist to evaluate patients at home, design individualized plans, and deliver hands‑on therapy. You’ll monitor progress, educate patients and families, and coordinate with the care team to ensure smooth transitions and optimal outcomes.

You will work autonomously in a home-based setting, building strong patient relationships and contributing to discharge planning with a focus on restoring mobility and independence.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s)
• Current PT license in the state of practice
• Minimum 1–2 years of experience (home health preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength and rehab potential
• Develop and implement individualized care plans with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance and independence
• Educate patients and caregivers on treatment plans, exercises and safety
• Monitor patient progress and adjust treatment plans as needed
• Coordinate with nurses, therapists and physicians for coordinated care
• Document patient care and progress accurately in the EMR
• Participate in case conferences and team meetings
